Typically, it is possible for a patient and his/her family to self-refer to our clinic.
If we need a referral from your primary care provider, we can often contact their office and obtain one for you.

Your Initial Visits To The Memory Clinic

Your New Patient Evaluation is a two-appointment process. Your first appointment will last about 3 hours and the second appointment about 1½ – 2 hours.

During your first appointment, you will participate in an interview where will we discuss any changes in your memory, ability to communicate, or behavior that you and/or your family members have noticed and are concerned about. Please note: It is very important that you bring someone who knows you well (like a spouse, friend, sibling, or child) with you to this appointment. You, the patient, will then work with one of our professional staff members who will have you perform various tasks that will help us to determine your cognitive strengths and weaknesses. These tasks are commonly referred to as a neuropsychological evaluation.

As needed, we will work with your primary care provider to obtain blood work and an imaging test (CT or MRI scan of the brain).

For your first appointment, please bring:

  • your new patient packet that you were sent in the mail.
  • your insurance cards
  • a list of medications, supplements and vitamins that you are currently taking. If you would like help with your medication list, please bring the bottles of all the medications, vitamins and supplements with you.

For your second appointment, you will have a thorough medical and neurological evaluation; during this evaluation, we will ask you to provide us with your medical history and any changes to your current medications. In addition, the results of your blood work and imaging scan will be reviewed with you and your family member(s).

At this point, your evaluation is complete and your diagnosis and treatment options have been determined by the treatment team.

We then meet with you and your family members to provide feedback on the entire evaluation, discussion of the diagnosis, and review of all available treatment options. The patient then selects the treatment options(s) that they are most interested in pursuing.

Why A Seahorse?

Many people ask us why we selected a seahorse for the logo of The Memory Clinic. The Latin word for seahorse is hippocampus, which is a part of the brain that is very important in memory. The term hippocampus was originally coined in 1587 by Julius Caesar Arantius because he thought that the anatomical structure of the hippocampus resembled a seahorse especially the curled structure of the tail!
