Arranging an appointment is easy!
Contact your primary care provider to discuss your memory concerns and request a referral to The Memory Clinic. We will need the medical history that comes with your referral, as well as blood work and imaging of your brain (an MRI or CT scan). We will work with your provider to obtain all the information needed to schedule your appointment.
Your Initial Visit To The Memory Clinic
For your appointment, please bring:
- your new patient packet that you were sent in the mail.
- your insurance cards
- a list of medications, supplements and vitamins that you are currently taking. If you would like help with your medication list, please bring the bottles of all the medications, vitamins and supplements with you
- Please note: Patients must bring someone who knows them well, like a family member or close friend, to participate in their appointments.
The entire new patient evaluation can take between 3–5 hours. When scheduling your appointment, we can let you know what to expect.
Prior to your appointment, we will work with your primary care provider to obtain blood work and an imaging test (CT or MRI scan of the brain).
During the first part of your appointment, you will participate in an interview where will we discuss any changes in your memory, ability to communicate, or behavior that you and/or your family members have noticed and are concerned about.
You, the patient, will then work with one of our professional staff members who will have you perform various tasks that will help us to determine your cognitive strengths and weaknesses. These tasks are commonly referred to as a neuropsychological evaluation.
During the second part of your appointment, you will have a thorough medical and neurological evaluation; during this evaluation, we will ask you to provide us with your medical history and any changes to your current medications. In addition, the results of your blood work and imaging scan will be reviewed with you and your family member(s).
We then meet with you and your family members to provide feedback on the entire evaluation, discussion of the diagnosis, and review of all available treatment options. The patient then selects the treatment options(s) that they are most interested in pursuing.
